I understand that my child may participate in games and activities at the LEAD Sports League Back‑to‑School Fest. I acknowledge that participation may involve normal physical activity and minor risks. I release LEAD Sports League, its staff, and volunteers from liability for any injuries or incidents that may occur, except in cases of gross negligence.

Medical Consent

I authorize LEAD Sports League staff or volunteers to provide basic first aid or seek emergency care for my child if needed. I understand that I am responsible for any medical costs.

Photo & Media Consent

I give permission for LEAD Sports League to use photos or videos taken at the event for promotional purposes.
